If you are a provider or practice staff, you need to log in via the practice login form. If you don't remember your password, click the "forgot your password?" link at the bottom of the form.
If you are a patient, you log in via your provider's patient portal. You may have different providers on DocMeIn and a different login (email and/or password) associated with each practice. You may also have a login that already exists with some of your providers, but not with all. Make sure you are using the login credentials you established with the practice you are attempting to log into.
